Respecting Choices® Advanced Steps
ACP Facilitator Certification
Advanced Steps (AS) ACP is initiated as a component of quality end-of-life care for frail elders and those whose death in the next 12 months would not be unexpected. The AS planning conversation is focused on goals of care to make timely, proactive, and specific end-of-life decisions. Ideally, these decisions are converted into medical orders that can be followed throughout the continuum of care.
Target Audience
The AS ACP Facilitator Certification course is intended for healthcare professionals(nurses, social workers, chaplains, physicians, advanced practice providers) and others involved in assisting those with advanced medical conditions and frailty in making treatment decisions based on their goals of care.
Purpose
The purpose of this program is to assist participants in learning effective communication and conversation skills and techniques for facilitating person-centered conversations for those living with advanced medical conditions and frailty. These conversations have application for in-person visits and telehealth.
Precourse Requirements/Activities
1. Complete Respecting Choices Advanced Steps Online ACP Facilitator Curriculum; modules 1-5(included in registration fee).
2. Review the Facilitator Handbook to Advanced Steps ACP Conversations.
Learning Outcomes
At the end of this course, the participant will be able to:
1. Identify communication skills for facilitating Advanced Steps ACP conversations.
2. Demonstrate beginning competency in facilitating Advanced Steps ACP conversations through role-play activities.
3. Create strategies to transfer individual goals, values and preferences to a written plan.
4. Become certified as an Advanced Steps ACP Facilitator through final competency role-play demonstration..
Initial Certification Requirements
Ongoing Certification Requirements
Currently, RC has no ongoing recertification requirements. It is recommended that recertification requirements be established and monitored by the Facilitator’s sponsoring organization and RC Instructors. RC intends to establish recertification requirements in the future.

The goal of this lesson is to provide the learner with an overview of the First Steps Advance Care Planning (ACP) facilitation process. This overview will include the basic components and goals of the process, as well as a description of the Respecting Choices® staged approach to ACP. Learners will also be introduced to the responsibilities they will have as an ACP Facilitator and the strategies they can use to create a written advance directive that is clear, accurate and complete.

The purpose of this lesson is to prepare individuals to become Advance Care Planning (ACP) Facilitators. The lesson provides an overview of advance care planning, outlines the first steps involved with making an advance care plan, provides learners with strategies for motivating individuals to make advance care plans, and presents a variety of scenarios and role-play opportunities that allow the learners to practice their facilitation skills.

The goal of this lesson is to provide the learner with strategies to incorporate First Steps Advance Care Planning (ACP) into the routine care management of adults with chronic illness. The learner will also be introduced to their responsibility to identify gaps in individuals’ knowledge and provide them with needed information and referrals. The lesson will also demonstrate how to use a decision-making framework to assist individuals in making informed treatment decisions and how to create strategies to ensure that the individual’s plan is followed.

The purpose of this lesson is to prepare individuals to become Advanced Steps Advance Care Planning Facilitators. This lesson provides an overview of the Advanced Steps ACP stage of planning, including the key features of a Physician Orders for Life-Sustaining Treatment (POLST) Paradigm program. It also provides learners with strategies to use when preparing for an Advanced Steps ACP conversation, as well as those to use during an Advanced Steps ACP conversation to ensure that an individual’s goals, values, fears and concerns are identified, they are adequately assisted in making treatment decisions consistent with their goals of care and that strategies are created to ensure that their plan is honored. Furthermore, this lesson provides learners with a variety of scenarios and role-play opportunities so that they can practice their facilitation skills.
