Our philosophy at the Jockey Club Home for Hospice (JCHH) is to provide continuing, seamless hospice and palliative care for our patients and their families, which is why we have integrated ambulatory and day services into our care model. Our team of experienced doctors, palliative care nurses and allied health professionals are committed to helping you control your symptoms so that you and your family can focus on spending quality time together.
Palliative care is a specialized form of care that helps you manage a serious, life-limiting health condition. It can be initiated at any stage of your illness to prevent or relieve suffering and improve quality of life for you and your family. Good palliative care is holistic, meaning your care team will treat you as a whole instead of focusing solely on your illness.
Your assigned home care nurse will serve as your case manager and main point of contact to the rest of your health care team. To arrange an appointment with us or one of your other service providers, simply contact your JCHH case manager who will coordinate your care for you. We will tailor your care to address your needs and the needs of those around you.
