You can consult with your case manager for advice 7 days a week, as necessary.

Professional at-home support means less trips to the hospital for uncontrolled symptoms. It also means you won’t have to worry as much about arranging transportation for your doctor’s appointments.
- Ease of mind for you and your family
Living with a serious illness is never easy; home care helps lighten your burden so you and your family can focus on spending quality time together.
Home care doesn’t mean you have to be at home to receive our care. We are more than happy to visit patients at aged homes or hospitals. If you are an existing in-patient at the Jockey Club Home for the Hospice, you are welcome to approach one of our in-patient nurses to learn more about our home care services.
