Care at home

Palliative home care is a specialized medical service for people with any advanced illness or life-limiting condition who wish to remain at home. As our home care patient, you will be assigned a case manager who is an experienced palliative care nurse and also your main point of contact to the rest of your health care team. Your case manager will visit you regularly to monitor your care and ensure that your physical and psychosocial needs are being met.
